Enrollment form for submitting applications via the National Registration Database ("NRD") Individual application forms are to be submitted via the National Registration Database. The firm must submit the enrollment form to CDS Inc. in order to obtain access to the NRD. Investor Application Form All individuals who wish to be salespersons, partners, officer, directors or shareholders holding 10% or more of the shares of the member applicant must also submit the Investor Application Form, as well as an NRD application, via the National Registration Database. Investor Notification Form To be completed by all investors purchasing shares of a non-public IIROC Member firm whose accumulated holdings are less than 10%.
